Register

Registering with the Hartenbank consists of two steps: signing up and registering. You sign up by requesting the information and consent forms.

If you have any questions after receiving the information and consent forms, please feel free to contact us at via info@hartenbank.nl or by telephone on 088-2333 699 (open Monday to Friday from 9 a.m. to 5 p.m.). If you have decided that you want to register as a future donor, please sign the consent forms and return them to the Heart Bank using the enclosed return envelope. If you do not return this consent form signed, you will receive a one-time reminder. If there is no response, your data will be deleted after 1 year.

Once the Hartenbank has received your fully completed consent forms, you will be registered in the Hartenbank’s donor database and will receive a codicil card. This card contains your details and the telephone number that your next of kin can use to inform the Hartenbank after your death.

The cooperation of people close to you is essential to ensure that the donation process runs smoothly. We therefore recommend that you discuss your decision to become a donor with them. The Hartenbank believes it is important that the decision to become a donor is made in close consultation with your loved ones.

When registering as a Hartenbank donor, you must provide the contact details of someone close to you. You are free to choose any adult you wish to designate for this purpose. This could be your spouse, partner, child, another relative or heir, a close friend, your general practitioner, or another trusted person of your choice. If we are unable to contact you and need to get in touch with you, we will contact your designated contact person to verify that your details are still correct.

Anyone over the age of 18 can register as a potential future Hartenbank donor. You do not need to have heart disease to do so. People with heart problems, people with a genetic predisposition to developing heart problems, but also healthy individuals (known as control donors) are invited to register as Hartenbank donors.
